Requirements:
- Application (Only available at a N.A. Information Session) - Do not do Drug Screen until you are instructed.
- Proof of immunity or immunizations ( list will be provided at NA Information Sessions listed above)
(also see Health & Safety Documents Q & A) - Health Declaration form filled out and signed by physician. (Form is included in the Nursing Assistant Application).
- CPR card - Curent Health Care Provider CPR/BLS or CPR Pro card - click here for more information
- Background Check & Drug Screen (information will be provided at your scheduled Application Appointment)
- Dept. of Public Safety Level One Fingerprint Clearance Card (See Fingerprinting Instructions below) - application is available at the Nursing Dept. Office, 2nd Floor of the Life Science Building on GCC's main campus (59th Ave & Olive).
