Dear Janine,
The Maricopa Institutional Review Board (IRB) reviewed your modification request for the protocol referenced on August 13, 2009. The modification to add additional MCCCD college sites beyond your college (SC) was approved for the colleges that indicated approval from the Vice Presidents of Academic Affairs subject to IRB review. Thus IRB approval applies to proposed research only at these three colleges, SC, GW and GC . (Should you receive approvals from PC and RS, please submit a modification request for additional sites.)
Your original determination has now been extended for one year from the date of this notification for SC, GW and GC. If your project continues beyond that date, it will be necessary for you to return again to the IRB for continued annual review. If you decide to make changes in your project design, you should seek IRB approval prior to making those changes.
The IRB Office will stamp IRB approval on your consent and assent forms and forward to you tomorrow. We appreciate your cooperation in complying with the federal guidelines that protect human research subjects. We wish you continued success in your project.
