Requirements:

  1. Completed Application
  2. Completed Health and Safety Documentation Checklist & Proof of immunity or immunizations for Tetanus, Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B, current TB testing
  3. Completed Health Declaration form from physician. (Form can be found in Basic Nursing Assistant Application).
  4. Current Health Care Provider CPR/BLS card
  5. Dept. of Public Safety Fingerprint Clearance Card (See Fingerprinting Instructions below) - application & instructions can be picked up at the Nursing Dept. Office, 2nd Floor of the Life Science Building on GCC's main campus (59th Ave & Olive). Note: It can take up to 8 weeks for DPS to issue a fingerprint clearance card.
