Applications for the October 2009 Nursing Assistant class will be accepted beginning August 24th in the Nursing reception area on the 2nd Floor of the Life Science building.

Admission to the N.A. Program is by application process only.

Requirements:

  1. Completed Application
  2. Completed Health and Safety Documentation Checklist (Proof of immunity or immunizations for Tetanus, Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B, current TB testing
  3. Completed Health Declaration form from physician. (Form can be found in Basic Nursing Assistant Application).
  4. Current Health Care Provider CPR/BLS card)
  5. Dept. of Public Safety Fingerprint Clearance Card (See Fingerprinting Instructions to the right)
