ADA Furniture Request Form

Campus Requesters: Use this form to initiate the request for furniture accommodations for disabled students in accordance with ADA requirements. This form will be used as a basic tool to gather information. Your submission will be forwarded to the Furnishings Management team and Disability Services & Resources.
