GCC Department Assessment Report Form

To be completed by the Department's Assessment Coordinator at the end of each school year and submitted to the Campus Assessment Coordinator or Dean of Instruction by June 30.

In addition, submit examples of at least three (3) completed course assessment examples from each semester. PLEASE COMPLETE ALL ASSESSMENTS ELECTRONICALLY.
